I hereby certify that an accredited Veterinarian has inspectted the above named dog on the inspection date of_______________amd that the dog is found to be healthy and sound there is no showing signs of infection , contagious and/or communicable diseases. All puppies receive their tails cropped, dewclaws removed, first shots and deworming before they leave the Sellers home.

To the best of my knowledge the dog listed above meets all state health requirerments, and is free of any genetic defects and conditions.

He/she should be fed 3 times a day with fresh water at all times.

We offer a 1yr health guarentee and for the protection of the buyer and seller the puppy must be examined by a licensed Vet within 72hrs of receving the puppy. If the puppy is found to be in unacceptable health within 72 hrs of receiving puppy, buyer may receive a full refund or exchange for another puppy when one is available. If puppy is found in unacceptable health you must submit a statement form a Vet clearly stating conditions found. If the puppy is not examined by a Vet within 72hrs, no health guarantee is made, no exchange will be given and no refund.

Seller will guarantee the puppy against life threatening hereditary conditions untill the puppy reaches one year of age. Proof from a Vet is required of hereditary condition puppy will be returned to the Seller within 7 days and he/she will be replaced as soon as another puppy comes available. If Buyer chooses to keep the puppy and treat it Seller will refund to Buyer 25% of fee's paid for adoption.

If puppy shall die before 1yr of age a autopsy must be performed within 24hrs of death. Seller must be immediately notified. Proof is required clearly stating cause of death. If hereditary cause, replacement puppy will be given as soon as available. Seller does not guarantee against internal or external parasites, worms or any minor treatable conditions, also all shots must be kept current. Seller does not guarentee against patellar luxation or tracheal collapse due to enviromental factors. No guarantee is given to size, color or temperament. once puppy reaches 1yr of age, no guarantee, exchanges or refunds will be given.

Buyer agrees that the above listed dog is not being adopted for the intent to breed. The Buyer understands that breeding smaller (toy and tea-cups) dogs can threaten the dogs overall health. Buyer agrees never to surrender this dog to a shelter. Buyer agrees to notify seller if it becomes necessary to rehome the dog.

Seller is not responsible for any Veterinarian fees, medications,testings, treatments and any other type of fees after the puppy leaves Seller. Shipping fees are non refundableand Seller is not responsible for any shipping costs if necessary.

This adoption and health agreement is non transferable. By signing below the Buyer agrees to honor everything stated above in this adoption and health agreement.
